I have a pain around both sides of my achilles tendon. I noticed it at
work first. It gradualy got worse during my shift and when I was
walking home, became very severe. I did not fall or lose balance or do
anything to cause this. (About a month ago, I also started having
pain in my left knee for no apparent reason. It went away in about
half a day, however, and was not as disabling as what I have now).

I could not sleep last night and had to take aspirins to relieve the
pain. In the morning, had to use a cane to get around. Severe pain
again on both sides of the achilles tendon but below the ankle bones
that jut out. It is swollen, hot and I also have a fever at this
point. No redness or any sign of "tophies," however.

I'v done Internet searches and I'm almost positive that it is gout.
It was real cold that day and I didn't wear proper clothes. I'm 40, a
bit overweight and have not been eating the best fare -- no organ
meats but plenty of red meat, corned beef, beans, lots of pasta,
spinach, broccoli, also lots of sodas -- used to drink 4 cans of
Welch's grape soda per day.

I had nephritis when I was 7 and have a history of above-average
blood pressure, high cholesterol levels and, for some reason, very
high liver enzyme levels -- a condition called "fatty liver."

Right now, almost 36 hours after the onset ... the pain has subsidies
somewhat. However, when I immobilize my foot, it begins to hurt again
when I try to move. I will take another day off and I'll get some
apple pectin (which a poster has recommended) and Ibuprofen from CVS.
Regarding NSAIDs, are they available over the counter? I'm seeking
immediate relief since my ankle feels like it's being torn apart by
tigers claws sometimes.

Also, it seems like I should go straight to a rheumatologist for this.
Or should I see my internist first? Should I just bypass foot and
ankle specialists and podiatrists? I have a PPO so I can go straight
to the specialist, although I have to satisfy about a thousand dollars
worth of deductible, which at this point I have no choice of avoiding.

I think that your PCP would be the best place to start...He would at least
know where you would be best benefitted...

i had a recent bout of gout. my gp was fine in treating it. he put me
on indocid/indomethycin, which is the most effective drug for getting
the urea crystals out of the joint. there is no reason to go to a
rheumatologist for immediate relief. after three days the pain was
gone, and the swelling was gone in another two days. i took the
indocid for five days total. (by the way, my body hates that drug, and
my stomach was upset all the time i was on it.)

however, once you know you have it, you may want to go to a specialist
to see if treatment is desirable in your case: something called
approximately cholchicine (don't hold me to that spelling) on a
continuing basis is used by real sufferers. i seem to be able to
treat it by diet.

no one knows why two people with the same density of urea in the
blood stream can exhibit varying outcomes - one healthy, the other
with gout.

in my case, it was age (i am 80) and the consequent reduction in
kidney function which probably led to the outbreak. much like type 2
diabetes whose onset occurs with age as insulin behavior
deteriorates with age.

the nurse who cuts my toenails discussed gout with me and said that
her father gets it in the ankle, as you did, instead of the toes as i
did.

Grayson was on colchisine and something else also for many years to keep his
from flaring. I can't remember what the other one was though.

An excessive amount of uric acid in the bloodstream because of diminished
kidney function is associated with gout. This (the uric acid present)
can be easily measured from a blood sample, along with other kidney function
tests. In my experience Colchicine is effective in relieving the gout and the
associated pain.
It commonly has unfortunate gastric side-effects (nausea, diarrhoea) and the
dosage must be monitored
carefully. For longer term management it would be best to see a rheumatologist,
as the drugs used and the dosages are individualised to a great extent. However,
this condition CAN be treated sucessfully.
