As a student at Champlain College in Burlington, Vermont, and
I am conducting a short survey which purpose is to identify gaps in
docter-patient relations, and to help empower patients in their search
for the right medical professional for their needs.

Oh, look. An "educational" survey designed to get lists of patients who have
problems with their doctors. What an amazingly useful tool for selling
advertising to potential medical customers interested in, say, colloidal
silver, ancient Eastern medical cures, and NONI JUICE.

If it's an educational survey, it should be hosted by your university.
Otherwise, there's a good chance it's a spammer advertising survey.
Remember, more and more spammers will now use addresses from "surveys" as an
excuse to spam you under the excuse of "they contacted *us*, see right here
on this form they filled out"?
