management of insulin and food management of insulin and food -- Posted by Isabelle Penman on 12-04-04 15:20
Does anyone have an idea how to manage food and insulin around an irregular
schedule? I have insomnia and I can literally be awake or asleep at any
hour of the day and night.
I talked to my Dr, and I dont think he understood at all. He got very
impatient with me when I told him I cant tell night from day and he said if
the sun is out "It's day. I often dont see the sun as I am asleep while it
is shining. If on a certain day I wake up at 6pm I take a shot (it is my
morning shot) How soon can I take the next shot? I think 8hours?
sometimes because of disturbed sleep I go 15 hr without a shot.
Anyone else have this problem? Any advice would be appreciated.

In my opinion, an insulin pump is the best bet for managing an irregular
schedule. You program the pump to continuously deliver a small amount of
basal insulin, so you don't need to worry about when to take your next
injection. The only time you need to manually control the pump is when you
need to take extra insulin--at meal time or if your BG is running high.
There are definitely some drawbacks to a pump, but it can't be beat for
helping to manage a flexible schedule.

I don't keep regular mealtimes either but a shot of Lantus once a day
handles my basal insulin. Then a small shot of Humalog at mealtimes
takes care of the rest.

yea, but you will probably need to change the types of insulin you are
using. what types are you currently using?

definitely not the right regime for a type 1.
so until you correct me I am going to assume you are a type 2 managing
with insulin. You would probably do better with lantus and might want
to consider with the help of an endocrinologist taking an anti
resistance med along with it. Lantus does not have the peak that NPH
has allowing you to avoid having to eat to the insulin to avoid hypos.
a type 1 should be taking both a long acting and fasting acting
insulin to properly cover their body's needs.
