OBESE MOTHERS MAY HAVE MORE FAT TISSUE THAT CAN CONSTRICT THE BIRTH
CANAL AND INCREASE THE INJURY RISK TO THEIR BABIES DURING DELIVERY.

Infants of obese women may be at greater risk for injury during
delivery than infants of normal-weight women. Stacy Colombo, a
fourth-year medical student at the University of Texas in Houston, led
a review of 202 cases of shoulder dystocia, a difficult birth in which
the baby's shoulder becomes stuck during delivery.

In 32 of these cases, the babies sustained injuries such as fractures
of the upper arm or clavicle, or a nerve injury called Erb's palsy in
which the baby is unable to move an arm.

When Colombo and her research team compared the shoulder dystocia cases
with a control group of 606 uneventful births, they found that obesity
in the mother was linked to an increased risk for injury to the baby
resulting from shoulder dystocia. The risk of Erb's palsy was
particularly associated with obesity.

Colombo says larger mothers may not only have larger babies, but may
also have more fat tissue that can constrict the birth canal and
increase the injury risk.

The study also examined the relationship between the incidence of Erb's
palsy and the number of personnel in the delivery room at the time of
the shoulder dystocia. The deliveries that resulted in Erb's palsy were
significantly more likely to have had only one nurse present.

"Typically, the more people you have to help alleviate the shoulder
dystocia, then you have less worry about risk of injury to the baby,"
Colombo says.

She recommends doctors think about ways to reduce the risk of injury
before an obese woman goes into labour -- particularly by having
additional personnel in the delivery room.
