G'mornin' everyone.
Currently I take 10 mg Glyburide and 500 mg Metformin ER 2x/day.
I also take 500 mg of Niacin 1x/day (Seems like a GREAT treatment
option + I'd like to not take another prescription drug). After
meeting with a nutritionist (N) yesterday, I'd like your
opinions.
I'm getting quite agitated that I'm not losing much weight
despite continued effort to count/watch every single carb I put
in my mouth and fairly consistent exercise (walking). My BGs
seem to be very reactive to carbs like rice and whole grains.
I'm probably averaging about 45 carbs/day which is low BUT my BGs
are good (usually FBG is around 90 and generally below 140 but
sometimes as low as 60 during the day). N suggested that Niacin
may be making BGs more reactive. She suggested that I take a
statin drug, increase Metformin and decrease the Glyburide. Now
I'm new to this whole game (diagnosed 5/27/05) but have a doc I
can talk to. Does this make sense to you? There seems to be a
lack of research (at least that I can find) which gives dosing
info.

Have you been counting calories? You could be eating too many of them even
if you are low carb. How much is 'much weight' and how fast are you
expecting to lose it, your expectations could be unrealistic. The more you
have to lose the more you would lose at the beginning.

The weight loss and diabetes group I have been with started by watching
primarily calories and fat. Limit of 1500 calories and 30 gr fat a day and
start with 30 minutes of exercise a day. I started at 320 pounds and got
down to 270 in a matter of a few weeks. It's amazing to see how much fat I
was consuming.

What is your total calories? Counting carbs is nice if it leads
to a reduction in total calories.

Don't know BJ. I am eating ALOT of cheese and higher fat foods
(peanut butter, mayo, etc) at the same time doubling my intake of
fresh veggies from the garden in order to fill me up. I'll start
trying to count calories and see.
So, what about Niacin- does it make BGs more reactive? I've just
changed to the insitol hexa form of Niacin.
Will increasing the Metformin and decreasing Glyburide help me
drop more weight? I'm constantly hungry without carbs to fill me
up so can't imagine cutting back the calories much more.

Glyburide can make you hungry and Metformin can make you less hungry. Be
sure you discuss with your doctor any changes you want to make.
I'm pretty sure I am eating way more than double the amounts of low carb
veggies as I was before. Way, way more.
ex. With a steak dinner I would eat a large baked potato and maybe a small
salad or small amount of canned veggie, maybe low carb, maybe not. Now I
eat a large salad and another low carb veggie and only part of the steak
because I am getting full.
